Case Report

This is a 28 years Para four mother on her third post partum period presented with sever persistent abdominal pain, vomiting, fever, fast breathing and failure to pass faces and flatus of three days duration. On physical examination Bp=80/40 mmHg T=38.8 PR=131bpm RR=32 bpm pale conjunctiva, grossly distended abdomen, sever direct and rebound tenderness, involuntary abdominal guarding and empty rectum on PR. investigation Hct=18% Bg and Rh=o+ PAF shows multiple air fluid level with distended colon. The Intraoperative finding were ileosigmoidal knotting (ileum were wrapped around the sigmoid) with gangrenous ileum and sigmoid. Untying the knot, resection of gangrenous ileum and sigmoid colon, ileo-transfers end to side anastomosis and end sigmoid colostomy was done Figures 1,2 & 3.

Current eradication rates achieved by the standard triple therapy are below 70%. Sequential therapy had provided a potential alternative that would compensate for the unmet performance of triple therapy. Despite its superiority to triple therapy, the excellent results reported initially were not reproduced by other investigators work. Adding adjuvant agents was also found to improve further the success rate of eradication. Probiotics emerged as a useful adjunct in treatment and prophylaxis of Helicobacter pylori infection. As opposed to other solid tumors, NETs represent a challenge, not only from the diagnostic standpoint but also in terms of treatment strategies. According to the data published by our Working group, ARGENTUM GROUP [ 1 ], with more than 600 patients assessed, roughly 60% of the cases present with metastatic disease from the onset. The reason why the incidence of advanced disease is high since early onset may be due to a bias, for most of the physicians in our Group are oncologists.
